OQP History Form- for collecting historical and anecdotal information about the quilt, its maker, family, and other noteworthy details. One set of forms should be completed for each item, and be completed before the Documentation Day.
OQP Physical Data Form- for collecting descriptive information about the quilt, such as size, colors, pattern, techniques, and materials. This will be completed by the Documentation Team.
Both of these forms are available right here, on the Oregon Quilt Project web site, and OQP committee members are available to help answer any questions.
